Popliteal lymph node dissection for metastases of cutaneous malignant melanoma

Popliteal lymph node dissection is performed when grossly metastatic nodal disease is encountered in the popliteal fossa or after microscopic metastasis is found in interval sentinel nodes during clinical staging of cutaneous malignant melanoma. Initially, an S-shaped incision is made to gain access to the popliteal fossa. A careful en bloc removal of fat tissue and lymph nodes is made to preserve and avoid the injury of peroneal and tibial nerves as well as popliteal vessels, following the previous recommendations. This rare surgical procedure was successfully employed in a patient with cutaneous malignant melanoma and nodal metastases at the popliteal fossa. The technique described by Karakousis was reproduced in a step-by-step fashion to allow anatomical identification of the neurovascular structures and radical resection with no post-operative morbidity and prompt recovery. Popliteal lymph node dissection is a rarely performed operative procedure. Following a lymphoscintigraphic examination of the popliteal nodal station, surgeons can be asked to explore the popliteal fossa. Detailed familiarity of the operative procedure is necessary, however, to avoid complications.

Driveclub coming to PS4 on October 7, celebrates with new trailer

Driveclub, a racing title for the Playstation 4, was first revealed last year with a fair amount of promise. Our own Seyth Miersma was intrigued, citing the game's competent physics, good looks and social features, during a short demo at the Electronics Entertainment Expo. Set for a position in the launch roster for the latest generation of Sony's gaming console, fans were disappointed when the game was unexpectedly delayed.

Now, we finally have a new launch date for the title - October 7, 2014. Yes, that's nearly a year after the Playstation 4 hit stores, but at this point, we're just happy to hear the game is arriving at all.

While racing gamers still have a few months to wait before they can sample the new title for themselves, Evolution Studios, the folks behind DriveClub, have released a new trailer made exclusively of in-game footage.

WoW Archivist: Talisman of Binding Shard, the lost legendary

This edition of WoW Archivist was originally published May 24, 2011. Given Blizzard's recent retrospective on Molten Core, we felt the piece of Warcraft history was worth another look. All references to time, space, and current content should be viewed through the lens of this piece's initial date of publication.

Last week, we finally escaped the morass of World of Warcraft's beta to discuss patch 1.2, the first major content patch of the post-release game. We're going to take a break from patches for a while to examine some other myths and legends that arose in vanilla WoW. Today, we're going to look back to one of the legends of Molten Core.

Molten Core is rather unique in that it's the home of more than one legendary item. Both Thunderfury and Sulfuras have their roots in Molten Core, though one does require items from Blackwing Lair to complete; Blackwing Lair hadn't even been implemented yet when players started receiving the first pieces of these legendary items.

Everybody knows about Thunderfury and Sulfuras, though. Not as many people know Molten Core once had a third legendary.

Blizzard on raid design evolution

Lead Game Designer Ion "Watcher" Hazzikostas has written a very extensive blog on the evolution of raid design, the first installment of which went live this morning. In this first part, Watcher covers the history of raiding, from the launch of the game in 2004 all the way through to the end of Icecrown Citadel in 2010.

For many, these were the glory days of raiding and World of Warcraft alike, well, if you believe the forums at least. Watcher talks about the developers' aims to make raiding more accessible, and to improve the gameplay of groups by reducing them in size -- one healer in a group of fifteen healers can't have as big an impact as one healer in a group of five or two. He also discusses the introduction of varying difficulties in raiding, and looks back over all the patches of some of the game's greatest raids.

Finding beauty in WoW's abandoned places

In a massively multiplayer game, you're almost always surrounded by crowds of other people. Your fellow players cluster around mailboxes and crowd around questgivers. They race you to monsters you need to kill and herbs you need to pick. In short, they're everywhere -- an inescapable part of playing an MMO.

Well, mostly. The A.V. Club has put together a writeup on WoW's abandoned spaces -- and despite the day to day MMO crowds, there are a lot of them. Have you visited Shattrath lately? What about Dalaran? These old content hubs are now abandoned by players, only inhabited by NPCs and the occasional alt-leveler. In fact, there are plenty of zones in the game where you can find you're the only person there -- and when you aren't crowded in by people, you can take a moment to enjoy the solitude and all the game details you might miss if you were racing a druid to the nearest ore node.

It's feigned solitude -- the game's other players are just a zone or a whisper away -- but it still works for a change of pace. For more of this quiet musing, just head out to one of WoW's abandoned spaces yourself -- or read through the full article on the A.V. Club.

Breakfast Topic: Professional

Let's play pretend for a moment: imagine an Azeroth that is at peace. The wars have ended, we've beaten all of the bad guys, and it's time to rebuild. Your class no longer matters. There's no need for violent adventurers, your skill in murder and mayhem is obsolete. You are now defined by your profession. Your profession is, in essence, your class. In such a scenario, what profession would you take up? What would you choose as your daily grind?

Let's assume secondary professions are included here. The fun option, in my opinion, would be archaeology. You'd still get to explore the world and have adventures! Realistically, though, I don't think I would mind being a full-time chef. Within the rules of Warcraft, cooks gather most of their own ingredients. As a player, I like killing things. In real life, I love gardening. Skilled cooking requires both, so why not be a chef? I even briefly considered attending culinary school. In Azeroth, maybe I'd do it.

What about you? Maybe you'd be a full-time angler. Heck, El already does it.

Nadir PSA level and time to nadir PSA are prognostic factors in patients with metastatic prostate cancer

Background:
Primary androgen deprivation therapy (PADT) is the most effective systemic therapy for patients with metastatic prostate cancer. Nevertheless, once PSA progression develops, the prognosis is serious and mortal. We sought to identify factors that predicted the prognosis in a series of patients with metastatic prostate cancer.
Methods:
Two-hundred eighty-six metastatic prostate cancer patients who received PADT from 1998 to 2005 in Nara Uro-Oncology Research Group were enrolled. The log-rank test and Cox's proportional hazards model were used to determine the predictive factors for prognosis; rate of castration-resistant prostate cancer (CRPC) and overall survival.
Results:
The median age, follow-up period and PSA level at diagnosis were 73 years, 47 months and 174 ng/mL, respectively. The 5-year overall survival rate was 63.0%. The multivariable analysis showed that Gleason score (Hazard ratio [HR]:1.362; 95% confidence interval [C.I.], 1.023-1.813), nadir PSA (HR:6.332; 95% C.I., 4.006-9.861) and time from PADT to nadir (HR:4.408; 95% C.I., 3.099-6.271) were independent prognostic factors of the incidence of CRPC. The independent parameters in the multivariate analysis that predicted overall survival were nadir PSA (HR:5.221; 95% C.I., 2.757-9.889) and time from PADT to nadir (HR:4.008; 95% C.I., 2.137-7.517). The factors predictive for overall survival from CRPC were nadir PSA (HR:2.988; 95% C.I., 1.370-6.518) and time from PADT to nadir (HR:3.611; 95% C.I., 1.462-8.921).
Conclusions:
Nadir PSA and time from PADT to nadir were factors that affect both CRPC and overall survival in a cohort of patients with metastatic prostate cancer. Lower nadir PSA level and longer time from PADT to nadir were good for survival and progression.

A loop-mediated isothermal amplification assay for the visual detection of duck circovirus

Background:
Duck circovirus (DuCV) infection in farmed ducks is associated with growth problems or retardation syndromes. Rapid identification of DuCV infected ducks is essential to control DuCV effectively. Therefore, this study aims to develop of an assay for DuCV to be highly specific, sensitive, and simple without any specialized equipment.
Methods:
A set of six specific primers was designed to target the sequences of the Rep gene of DuCV, and A loop-mediated isothermal amplification (LAMP) assay were developed and the reaction conditions were optimized for rapid detection of DuCV.
Results:
The LAMP assay reaction was conducted in a 62[degree sign]C water bath condition for 50 min. Then the amplification products were visualized directly for color changes. This LAMP assay is highly sensitive and able to detect twenty copies of DuCV DNA. The specificity of this LAMP assay was supported by no cross-reaction with other duck pathogens.
Conclusion:
This LAMP method for DuCV is highly specific and sensitive and can be used as a rapid and direct diagnostic assay for testing clinical samples.

As the Dallas Cowboys prepare for the 2014 NFL Draft, there are no shortage of mock drafts projecting which direction they’ll head in the first round. It wasn’t long ago that there was one name sitting atop the list of Cowboys mock drafts—Pitt defensive tackle Aaron Donald.

Even after the signing of free-agent defensive tackle Henry Melton, though, another defensive tackle has replaced Donald as the most popular selection for Dallas in mock drafts—Florida State’s Timmy Jernigan. Of the mock drafts I assessed, Jernigan’s name appears in Dallas’ draft slot more than any other, by a wide margin.

So is there value in studying mock drafts? On the individual level, the value comes in dissecting one potential path the draft could follow. The real value, though, comes when we study multiple mock drafts. When an individual expert projects a player to a team, it doesn’t mean all that much. When a second one does it, that means a little bit more, and a third means a little bit more, and so on. Thus, something that isn’t terribly useful on the individual level has a lot of value when we aggregate the results.

With that said, let’s take a look at the most popular players being sent to Dallas in mock drafts, each of which I’ve assigned a “likelihood rating.” These subjective ratings are 1 to 10, reflecting how probable it is that each particular prospect will get drafted by Dallas.

UFC 172 Results: Breaking Down Jon Jones' Victory and More Top Storylines

UFC 172 made history thanks to the dominance of Jon “Bones” Jones against Glover Teixeira. 

ESPN Stats & Info broke it down for fans:

Let’s take a look at his impressive victory, as well as some of the other results from UFC 172.

 

Jones Dominates

Jones improved his record to 20-1 and was never challenged by Teixeira over the course of five rounds in Baltimore. All three judges scored the contest in favor of Jones, 50-45, in what was a much more dominating performance than his last fight against Alexander Gustafsson.

Jones commented on the fight afterward, via Brett Okamoto of ESPN.com: "It was a lot of improv. Game plan was to stick takedowns and pick him apart from a distance. I realized he was winding up on his punches so I switched game plans and fought at extremely close range. It worked out great."

Jones controlled the entire fight with his 84” reach and never gave Teixeira much of a chance.

In fact, the fans even started to boo Teixeira in the fifth round when his mouthpiece fell out.

After such a dominating performance, it’s only natural for Jones to look ahead, especially since he nearly lost to Gustafsson in his previous bout.

Of particular interest is the fact that the UFC is considering taking that fight to Sweden.

Considering how close the Swede came to knocking off Jones in their previous fight, any edge that the home crowd provides would be huge. Gustafsson would likely come out energized and perhaps even take control of the match in the early going. 

However, crowd support can’t do anything about Jones’ reach and overall athletic ability. He will learn from his last fight with Gustafsson and likely add onto his historically great resume in the process.

 

Luke Rockhold Impresses

Jones’ victory was the headline of Saturday’s action, but Luke Rockhold looked dominant as well when he made quick work of Tim Boetsch.

In fact, Rockhold notched a victory in slightly more than two minutes, which was one of the fastest wins of his career. The magnitude of the stage gave the former Strikeforce middleweight champion a sizable audience to impress, and that is exactly what he did. 

Rockhold thrilled fans by submitting Boetsch via kimura on the way to an easy win.

 

Anthony Johnson Destroys Phil Davis

In the fight before the main event, Anthony Johnson gave fans a taste of what they were about to see from Jones when he destroyed Phil Davis. 

Johnson is a 205-pound specimen who mixes power and wrestling ability better than the majority of fighters in the sport. Gregg Doyel of CBS Sports deemed Johnson’s performance to be a beatdown:

Johnson won 30-27 on every scorecard and stuffed all eight of Davis’ takedown attempts.

Breakfast Topic: Do you still PUG?

The idea of playing in a PUG -- or a random pick up group -- was once a painful one. You might spend ages in chat trying to find random players willing to join you for a dungeon only to get stuck spending hours trying to successfully navigate the dungeon... or even longer if you were unlucky enough to be trying to run Blackrock Depths. Grouping could be painful, which meant you tended to stick with guildmates or friends for dungeon runs that wouldn't leave you wanting to tear your hair out.

Of course, in today's game, the dungeon finder and raid finder make getting groups together a simple task, and dungeon running itself has been streamlined to make it easier for random groups to succeed. With these changes, it's gotten a lot easier to run a random dungeon, and so some of the stigma around PUGs seems to have faded -- or perhaps just transferred to LFRs.

For today's discussion, I'm wondering: do you play WoW in random groups? Or do you rather stick with guildmates and friends?

Got a really good question I want to answer off the bat, so here we go.

what's one thing I should show my sister at level 60? This is her first time through the game. Any other don't miss points?

There's actually a lot of stuff I wouldn't want to miss in the game at level 60, but if you're forcing me to choose but one, I'd say take her to the various raid zones - Blackrock Mountain, Zul'Gurub, Ahn'Qiraj - from back in Vanilla days. Get her to turn her distances all the way up so she can really see them, too. There was a lot of work put into these areas back when they launched, and especially Blackrock Mountain is always going to be one of my favorite places. I still go in and run around inside the various dungeons and raids from time to time, because no place in WoW has ever come as close to feeling like a real place people live in and fight over to me, while also feeling absolutely unreal and fantastic. It's built right into a volcano. I know it's probably nostalgia, but I find Blackrock Mountain amazingly cool. Don't forget the Anduin Lothar statue outside, either.

Remember, they're updating UBRS for Warlords, so go see it while you can.

Mini Paceman Adventure photos arrive by the trucklette-load

Mini Paceman

Yesterday, we brought you the news of the existence of this Mini Paceman Adventure tiny truck, and the story of how it came to be. Long story short: it was a lighthearted collaboration between apprentices and instructors at parent BMW plants in Germany. The mini-sized pickup bed and off-road kit may not make the Paceman All4 actually practical, but we'd argue that they do make the one-off fairly loveable.

Apparently the Mini photographers agree with that "loveable" part. We'd heard that there were some better, higher-resolution images of the Paceman Adventure on the way, but we didn't expect there to be hundreds of them. But as you can see, the shooters went out and thoroughly documented the pickup, shooting it in several locations, still and moving, and documenting some of its construction as well. Long-term effects of three Tiao-Bu Fei-Shen therapies on NF-kappaB/TGF-beta1/smad2 signaling in rats with chronic obstructive pulmonary disease

Background:
The three Tiao-Bu Fei-Shen (Bufei Jianpi, Bufei Yishen, Yiqi Zishen) granules have been confirmed for their beneficial clinical efficacy in chronic obstructive pulmonary disease (COPD) patients on reducing frequency and duration of acute exacerbation, improving syndromes, pulmonary function and exercise capacity. But the short- or long-term mechanism of them is not fully clear. Nuclear factor (NF)-kappaB/transforming growth factor (TGF)-beta1/smad2 signaling pathway is involved in the progress of inflammation and remodeling in chronic obstructive pulmonary disease COPD. This study aimed to explore the long-term effects mechanism of Tiao-Bu Fei-Shen granules by regulating NF-kappaB/TGF-beta/Smads signaling in rats with COPD.
Methods:
Sprague Dawley rats were randomized into control, model, Bufei Jianpi, Bufei Yishen, Yiqi Zishen and aminophylline groups. COPD rats, induced by cigarette smoke and bacterial infections exposures, were administrated intragastricly by normal saline, Bufei Jianpi, Bufei Yishen, Yiqi Zishen granules or aminophylline from week 9 through 20, respectively. At week 20 and 32, lung tissues were harvested. Immunohistochemistry was used to detect interleukin (IL)-1beta and tumor necrosis factor (TNF)-alpha, quantitative real-time polymerase chain reaction (qRT-PCR) was used for TGF-beta1 and Smad2 mRNA analysis, western blotting was used to determine the phosphorylation of NF-kappaB (p-NF-kappaB) and IkappaBalpha (p-IkappaBalpha).
Results:
COPD rats had marked airway injury, such as chronic airway inflammation and remodeling, emphysema, which were improved in the three traditional Chinese medicines (TCM)-treated animals. The levels of IL-1beta, TNF-alpha, p-NF-kappaB, p-IkappaBalpha, TGF-beta1 and Smad2 were significantly higher in COPD rats than in controls, while they were dramatically reduced in the three TCM- and aminophylline-treated groups. At the meantime, all these endpoints were significantly lower in three TCM-treated groups than in aminophylline group, especially in Bufei Jianpi and Bufei Yishen groups. Compared to week 20, all endpoints decreased significantly in three TCM groups at week 32.
Conclusion:
The three Tiao-Bu Fei-Shen therapies can reduce pulmonary inflammation and remodeling in COPD and have significant long-term effects. NF-kappaB/TGF-beta1/smad2 signaling might be involved in the mechanism.

Are you playing Hearthstone for iPad?

I admit, I was charmed by Hearthstone during its beta. But after a lot of playtime -- and the beta wipe which cleared my card collection -- the game lost some of its shine. By launch, I was playing very little, if at all, and my gametime has dwindled ever since. The Aston Martin V12 Vantage S is a monster of a sports coupe. At the front, those color-matched fangs and open mouth look like Dracula ready to take a bite. And underneath the hood, the 6.0-liter V12 sounds like the Wolf Man growling whenever the driver gets on the throttle. In a recent video, XCAR got its hands on this wonderful behemoth and showed its brilliant brutality.

The smartest move that the producers made was letting the car speak for itself. Several times during the video, the host shuts up, and the only sound is the bellow from the exhaust of the Aston Martin. The cinematography is pretty darn nice, too. If you don't have time to watch the whole video, skip about 10 minutes into it when XCAR just lets the diabolical V12 thunder. It's definitely one of the highlights.
