Using new technologies to promote weight management: a randomised controlled trial study protocol

Background:
Over the last three decades, overweight and obesity and the associated health consequences have become global public health priorities. Methods that have been tried to address this problem have not had the desired impact, suggesting that other approaches need to be considered. One of the lessons learned throughout these attempts is that permanent weight loss requires sustained dietary and lifestyle changes, yet adherence to weight management programs has often been noted as one of the biggest challenges. This trial aims to address this issue by examining whether social media, as a potential health promotion tool, will improve adherence to a weight management program. To test the effectiveness of this measure, the designated program will be delivered via the popular social networking site Facebook, and compared to a standard delivery method that provides exactly the same content but which is communicated through a pamphlet. The trial will be conducted over a period of twelve weeks, with a twelve week follow-up. Although weight loss is expected, this study will specifically investigate the effectiveness of social media as a program delivery method. The program utilised will be one that has already been proven to achieve weight loss, namely The CSIRO Total Wellbeing Diet.Methods/designThis project will be conducted as a 3-arm randomised controlled trial. One hundred and twenty participants will be recruited from the Perth community, and will be randomly assigned to one of the following three groups: the Facebook group, the pamphlet group, or a control group. The Facebook Group will receive the weight management program delivered via a closed group in Facebook, the Pamphlet Group will be given the same weight management program presented in a booklet, and the Control Group will follow the Australian Dietary Guidelines and the National Physical Activity Guidelines for Adults as usual care. Change in weight, body composition and waist circumference will be initial indicators of adherence to the program. Secondary outcome measures will be blood glucose, insulin, blood pressure, arterial stiffness, physical activity, eating behaviour, mental well-being (stress, anxiety, and depression), social support, self-control, self-efficacy, Facebook activity, and program evaluation.DiscussionIt is expected that this trial will support the use of social media - a source of social support and information sharing - as a delivery method for weight management programs, enhancing the reduction in weight expected from dietary and physical activity changes. Facebook is a popular, easy to access and cost-effective online platform that can be used to assist the formation of social groups, and could be translated into health promotion practice relatively easily. It is anticipated in the context of the predicted findings that social media will provide an invaluable resource for health professionals and patients alike.Trial registrationAustralian New Zealand Clinical Trials Register (ANZCTR): ACTRN12614000536662. 2015 NBA Mock Draft: Latest Predictions for 1st-Round Prospects

The 2015 NBA draft is still approximately a month away, but with some time to process the ramifications of the draft lottery, it's worth revisiting how the first round might play out on June 25.

Although mock drafts can't really account for trades, there doesn't figure to be a lot of maneuvering toward the top of the order. The consensus seems pretty strong with regard to who the top prospects are.

For example, the Minnesota Timberwolves have a loaded cast of young perimeter players and seem highly likely to take either Karl-Anthony Towns or Jahlil Okafor with the No. 1 overall pick. Yours truly believes Towns will be the first man off the board in the following mock scenario:

Towns is an ideal fit in the Twin Cities, because he has the skill set to play power forward, unlike Okafor, who is anchored in as a traditional center.  

Interior defense is an area the Timberwolves struggle with, partly because of Nikola Pekovic's inability to stay healthy and ineffectiveness on that end of the floor when he is in the lineup. Minnesota needs a rim protector and versatile, rangy frontcourt defender of Towns' caliber to help the cause.

A young nucleus featuring Andrew Wiggins on the wing, Zach LaVine and Ricky Rubio in the backcourt and Towns up front would be formidable. It could even help the Timberwolves turn around more quickly than anticipated.

This would leave the Los Angeles Lakers to take Okafor, forming an offense-heavy front line of the future with Julius Randle. Duke swingman Justise Winslow may be a tantalizing option for L.A. and perhaps the best Blue Devil to fit the team's current personnel.

But once legendary guard Kobe Bryant retires, the front office will have an enormity of freedom to build the roster. Bleacher Report's Howard Beck reported what Lakers general manager Mitch Kupchak said recently about Bryant:

Okafor has the most upside of perhaps anyone on the board, with an amazingly polished post game and plenty of ability to be a perennial All-Star. Joining the ultra-competitive Bryant, who will be pining for one last great playoff run, will only help Okafor tap into his potential.

The next debate figures to be about which perimeter player will be first to be drafted. It's between either Emmanuel Mudiay or D'Angelo Russell, and the latter seems destined for the Philadelphia 76ers.

Philly has been rebuilding and enduring a lot of growing pains over the past two seasons. It's time to show some results, especially in a rather weak Eastern Conference. Adding Russell to the mix to be the offensive catalyst is the best possible move.

Zach Harper of CBSSports.com is already pondering whether Russell doesn't deserve to be selected above the two big men:

Russell oozes with confidence, has swagger to his game and is effective off the ball to go with rare floor vision and passing ability. Dynamic as Mudiay can be in the open court with his superior athleticism, the safer pick is Russell, whose attributes bring to mind current NBA MVP Stephen Curry.

Much like the 2014 draft, where most experts had Wiggins, Jabari Parker and Joel Embiid rated as the top players, the trio to headline this year's class figures to be Towns, Okafor and Russell, barring something absolutely wild.

Those three figure to be legitimate franchise cornerstones, players to be the face of an organization's future that are so hard to find later in the draft. It's important to nail the early picks when the opportunity arrives, but the teams at the top all have logical fits and seemingly can't-miss players to acquire.

Economic evaluation of Community Level Interventions for Pre-eclampsia (CLIP) in South Asian and African countries: a study protocol

Background:
Globally, hypertensive disorders of pregnancy, particularly pre-eclampsia and eclampsia, are the leading cause of maternal and neonatal mortality, and impose substantial burdens on the families of pregnant women, their communities, and healthcare systems. The Community Level Interventions for Pre-eclampsia (CLIP) Trial evaluates a package of care applied at both community and primary health centres to reduce maternal and perinatal disabilities and deaths resulting from the failure to identify and manage pre-eclampsia at the community level. Economic evaluation of health interventions can play a pivotal role in priority setting and inform policy decisions for scale-up. At present, there is a paucity of published literature on the methodology of economic evaluation of large, multi-country, community-based interventions in the area of maternal and perinatal health. This study protocol describes the application of methodology for economic evaluation of the CLIP in South Asia and Africa.
Methods:
A mixed-design approach i.e. cost-effectiveness analysis (CEA) and qualitative thematic analysis will be used alongside the trial to prospectively evaluate the economic impact of CLIP from a societal perspective. Data on health resource utilization, costs, and pregnancy outcomes will be collected through structured questionnaires embedded into the pregnancy surveillance, cross-sectional survey and budgetary reviews. Qualitative data will be collected through focus groups (FGs) with pregnant women, household male-decision makers, care providers, and district level health decision makers. The incremental cost-effectiveness ratio will be calculated for healthcare system and societal perspectives, taking into account the country-specific model inputs (costs and outcome) from the CLIP Trial. Emerging themes from FGs will inform the design of the model, and help to interpret findings of the CEA.DiscussionThe World Health Organization (WHO) strongly recommends cost-effective interventions as a key aspect of achieving Millennium Development Goal (MDG)-5 (i.e. 75 % reduction in maternal mortality from 1990 levels by 2015). To date, most cost-effectiveness studies in this field have focused specifically on the diagnostic and clinical management of pre-eclampsia, yet rarely on community-based interventions in low-and-middle-income countries (LMICs). This study protocol will be of interest to public health scientists and health economists undertaking community-based trials in the area of maternal and perinatal health, particularly in LMICs.Trial registrationClinicalTrials.gov: NCT01911494

Indy 500 Results 2015: Finishing Order and Top Drivers from Sunday's Race

We'll put it this way: Juan Pablo Montoya certainly took a circuitous route to winning his second Indianapolis 500.

Montoya passed teammate Will Power with three laps remaining and held him off the rest of the way, kissing the bricks for his second time in three races at the hallowed speedway. Of course, Montoya's first victory came all the way back in 2000. His 15-year gap between wins is the biggest in history.

"That was fun," Montoya said, per Mark Glendenning of Autosport. "I knew I had a good car when I came through the field. But the fight at the end was hard. It's awesome. I'm looking for the words; it's too much."

Power, who battled all day near the top of the leaderboard, finished second. Charlie Kimball, Scott Dixon and Graham Rahal rounded out the top five. Starting on the pole, Dixon was dominant for most of the race, leading 84 laps and doing his best to fend off the two Team Penske cars at his heels.

Dixon's car sputtered late, particularly on turns. He was barely able to stay in the top five after the final restart due to consistent problems with the car. 

"We didn't have enough speed. We kind of went back and forth on ignition settings," Dixon told reporters, via The Associated Press' Dan Gelston. "The car was overheating a bit and just too much understeer is what it came down to."

Tony Kanaan and Ed Carpenter were among the notable drivers who got into trouble. Kanaan led 30 laps before being involved in a crash on Lap 153. He spun into the wall and wound up finishing 26th. Carpenter went into an Indianapolis wall for the second time in three weeks and finished 30th. 

Montoya, meanwhile, found the right settings at the best possible time. The No. 2 car led just nine laps on the day. He got past Power with three to go and wound up taking the race by .1046 seconds, one of the closest results in history.

"You give that guy the bit and put it in his mouth and he doesn’t give up," Penske said, per Dave Lewandowski of IndyCar.com. "It’s a great day for Team Penske. I knew we had two up there, but the worry was Dixon and the 83 (Charlie Kimball). At the end of the day, they played fair. Good passing and we won the race.”

The win proved a stark contrast to Montoya's first Indy 500 triumph. In 2000, Montoya led 167 of the 200 laps. It was among the more dominant performances in race history and came during a period of international ascent for the Colombian. From 1999-2006, Montoya took 18 open-wheel races and became known as perhaps the best all-around driver in the world.

That dominance sparked an invite to make the transition to Sprint Cup, where he spent nine largely unsuccessful seasons. He returned to the open-wheel format last year when he joined Penske, a move that's already paying huge dividends. Sunday's win was his third since joining the IndyCar Series and second of the 2015 season.

Montoya currently leads Power by 25 points in the standings. He's the only driver with multiple victories this season and has finished inside the top five in five of the six races thus far. After an up-and-down decade-and-a-half, it appears Montoya's using the Indy 500 to once again reclaim his status as the best open-wheel racer on the planet.

"2000 was the start of my career," Montoya said, per Autosport. "I was really young. We came here, had a really good car, we dominated. That was an easy race. But this was a lot of work. When you have to work for it that hard, it's exciting."

The task now will be carrying that momentum over for the remainder of 2015. The IndyCar Series heads to Detroit next for the Dual over the weekend. Last year, Montoya finished 12th and 13th, respectively, in those races and could never quite figure out the track.

But after that, the schedule heads to Montoya's bread and butter. He earned top-five finishes at Fontana, Texas and Milwaukee last season. Each race is an opportunity for him to get a third win and extend his points lead even more.

Given his stellar performance Sunday, we may have witnessed the true beginning of Montoya's first IndyCar championship run.

Weighted protein residue networks based on joint recurrences between residues

Background:
Weighted and un-weighted protein residue networks can predict key functional residues in proteins based on the closeness centrality C and betweenness centrality B values for each residue. A static snapshot of the protein structure, and a cutoff distance, are used to define edges between the network nodes. In this work we apply the weighted network approach to study the ?-Lactamase Inhibitory Protein (BLIP). Joint recurrences extracted from molecular dynamics MD trajectory positions of the protein residue carbon alpha atoms are used to define edge weights between nodes, and no cutoff distance is used. The results for B and C from our approach are compared with those extracted from an un-weighted network, and a weighted network that uses interatomic contacts to define edge weights between nodes, respectively.
Results:
The joint recurrence weighted network approach performs well in pointing out key protein residues. Furthermore, it seems to emphasize residues with medium to high relative solvent accessibility that lie in loop regions between secondary structure elements of the protein.
Conclusions:
Protein residue networks that use joint recurrences extracted from molecular dynamics simulations of a solvated protein perform well in pointing to hotspot residues and hotspot clusters. This approach uses no distance cutoff threshold, and does not exclude any interactions between the residues, including water-mediated interactions.

Muscle strength and areal bone mineral density at the hip in women: a cross-sectional study

Background:
Muscle strengthening exercises are promoted for building and maintaining a healthy skeleton. We aimed to investigate the relationship between muscle strength and areal bone mineral density (BMD) at the hip in women aged 26?97 years.
Methods:
This cross-sectional study utilises data from 863 women assessed for the Geelong Osteoporosis Study. Measures of hip flexor and abductor strength were made using a hand-held dynamometer (Nicholas Manual Muscle Tester). The maximal measure from three trials on each leg was used for analyses. BMD was measured at the hip using dual energy x-ray absorptiometry (DXA; Lunar DPX-L). Total lean mass, body fat mass and appendicular lean mass were determined from whole body DXA scans. Linear regression techniques were used with muscle strength as the independent variable and BMD as the dependent variable. Models were adjusted for age and indices of body composition.
Results:
Measures of age-adjusted hip flexor strength and hip abductor strength were positively associated with total hip BMD. For each standard deviation (SD) increase in hip flexor strength, the increase in mean total hip BMD (SD) was 10.4�% (p?=?0.009). A similar pattern was observed for hip abductor strength, with an increase in mean total hip BMD of 22.8�% (p?=?0.025). All associations between hip muscle strength and total hip BMD were independent of height, but were nullified after adjusting for appendicular lean mass or total lean mass.
Conclusions:
There was a positive association observed between muscle strength and BMD at the hip. However, this association was explained by measures of lean mass.

A porcine reproductive and respiratory syndrome virus (PRRSV) vaccine candidate based on the fusion protein of PRRSV glycoprotein 5 and the Toll-like Receptor-5 agonist Salmonella Typhimurium FljB

Background:
Porcine reproductive and respiratory syndrome (PRRS) is characterized by severe reproductive failure and severe pneumonia in neonatal pigs and is caused by PRRS virus (PRRSV). Glycoprotein 5 (GP5) from PRRSV is a key inducer of neutralizing antibodies. Flagellin, a toll-like receptor 5 (TLR-5) agonist, is an effective inducer of innate immune responses. This study presents a novel PRRSV vaccine candidate based on the adjuvant effect of Salmonella Typhimurium FljB fused with PRRSV GP5.
Results:
A truncated rGP5 gene lacking the signal peptide and transmembrane sequences was amplified and inserted into prokaryotic expression vectors, pColdI or pGEX-6p-1. Salmonella Typhimurium flagellin fljB was amplified and inserted into the plasmid pCold-rGP5, generating recombinant plasmid pCold-rGP5-fljB. Histidine (His)-tagged rGP5 and fusion protein rGP5-FljB were induced with isopropyl-?-d-thiogalactoside, verified by SDS-PAGE and western blotting, and purified via Ni-NTA affinity columns. The TLR-5-specific bioactivity of fusion protein rGP5-FljB was determined by detecting the expression levels of the cytokine IL-8 in HEK293-mTLR5 cells by sandwich ELISA. The purified endotoxin-free proteins were administered intraperitoneally in a C3H/HeJ mouse model. The results show that immunization with the fusion protein rGP5-FljB induced a significantly enhanced GP5-specific and PRRSV-specific IgG response that persisted for almost 5�weeks. Co-administration of the rGP5 with R848 or Alum also yielded a higher IgG response than administration of rGP5 alone. The IgG1/IgG2a ratio in the rGP5-FljB immunization group was significantly higher (9-fold) than that in the rGP5 alone group and was equivalent to the response in the rGP5?+?Alum immunization group, suggesting a strong Th2 immune response was induced by the fusion protein.
Conclusions:
Purified fusion protein rGP5-FljB is capable of activating the innate immune response, as demonstrated by the results of our TLR-5-specific bioactivity assay, and FljB has adjuvant activity, as shown by the results from our administration of rGP5-FljB in a mouse model. Our findings confirm that FljB could serve as an excellent adjuvant for the production of GP5-specific and PRRSV-specific IgG antibodies as part of an induction of a robust humoral immune response.

Why Alejandro Grimaldo Will Be the Next Great Product from Barcelona's La Masia

Alejandro Grimaldo has been well known to Barcelona connoisseurs for years now.

Ever since he burst onto the scene as the youngest-ever Barca B graduate at 15 years and 349 days against Cartagena in the Segunda Division, Grimaldo has been destined for greatness.

A left-back by trade, the youngster has a style which is very easy on the eye and a maturity that far belies his tender years.

He may not have the sharp burst of speed as his contemporary in the position, Jordi Alba, but his crossing ability, passing accuracy and innate ability to read the game are from a player that operates at the very highest level.

Although many at the club and beyond talk about the emergence of Sergi Samper, Alen Halilovic and Adama Traore, all of those players will struggle to break into the side whilst the attacking and midfield berths are taken by the likes of Lionel Messi, Sergio Busquets, Andres Iniesta et al.

That's not to say that they won't ever pull on the Blaugrana in earnest, it just might not be as quick as supporters expect.

Grimaldo on the other hand has a genuine chance, providing he remains free of injury, of breaking into the first-team squad and giving Alba a real run for his money on that left side.

For example, Alba is frequently caught out of position but his speed in getting back often covers for his initial incompetence.

Grimaldo is a much more considered player who doesn't fly forward at the drop of a hat. With defence very much in mind, he will gamble further upfield when possible but only if he doesn't leave himself exposed at the back.

Should he station himself in a wide attacking berth, you can be assured that any delivery into the box or short passes where required, will be executed with accuracy and aplomb.

 

Luis Enrique has already drafted him in for first-team duties this season, Grimaldo playing in the Catalan Cup against Espanyol, a trophy which Barcelona won on a penalty shootout, one of which was coolly dispatched by Grimaldo.

No matter the assignment, Grimaldo takes it all in his stride, nothing seems to faze him. Ever. 

That is a super quality for a player to possess and no doubt the manager has already taken note of the same.

His football grounding hasn't been exclusively Barca-led given that he was at Valencia for two years before joining the Blaugrana at just 13 years of age.

 

But to that end however, he already has six years of experience of life at this monumentally huge Spanish institution, and the pressures therein.

His performances at all levels of La Masia have already seen him take international honours from the U16 level of Spain's national side, right through to the U21's. Well deserved call-ups they are too.

There will be no surprises for this most studious of operators when he is eventually called upon for regular first-team action, and it will be no surprise to anyone else if Grimaldo is introduced into the first team at some point during the 2015/16 season.

The only question mark that could possibly prevent the same would be a longer-than-usual recovery from his latest injury which required him to leave the field on a stretcher as seen above.

Big Ten Baseball Tournament 2015: Thursday Schedule, Predictions and More

The Big Ten is known for its strength on the football field and basketball court, but if Wednesday’s baseball games were any indication, the rest of the conference tournament is appointment viewing for hardball fans.

Iowa’s Nick Day stunned Ohio State with a dramatic walk-off home run. Illinois, the overwhelming favorite, needed some late magic to rally past underdog Nebraska. Maryland edged Michigan State in the nightcap. The two-time defending Big Ten tournament champion Indiana Hoosiers lost to Michigan.

Thursday is when the real drama starts, as Wednesday's losers will fight for their tournament lives, and the winners will attempt to create separation from the rest of the field in the double-elimination format.

With that in mind, here is a look at Thursday’s schedule, predicted winners and game to watch.

 

Thursday’s Game to Watch: No. 1 Illinois vs. No. 4 Maryland

With all due respect to Maryland, this is the game to watch because it features powerhouse Illinois.

The Fighting Illini went 21-1 during the Big Ten regular season on the way to a conference crown and won their 27th game in a row Wednesday. That tournament-opening victory came against Nebraska, and Illinois' official Twitter account passed along some of the history the Fighting Illini made in the process:

Simply put, Illinois is a machine and the clear favorite to capture the tournament crown. It will likely host an NCAA tournament regional as well, although winning the Big Ten tournament would go a long way toward solidifying what is already a sparking resume.

For as great as the Fighting Illini have been all season, they didn't look unbeatable Wednesday against the Cornhuskers. In fact, Nebraska seized the initial 2-0 lead, and Illinois needed a Pat McInerney single in the bottom of the eighth inning to complete the comeback on the way to a 3-2 win.

While a stronger team than Nebraska may have beaten Illinois' Wednesday effort, the pitching staff did its job and held the Cornhuskers to two runs. Kevin Duchene started the game, and superstar closer Tyler Jay demonstrated why Illinois will be so difficult to beat in this tournament.

He shut the door in three batters on the way to his 23rd save of the year. If the opponent isn't tied or leading after eight, the game is over with Jay on the mound. That is a problem for the rest of the league if it hopes to somehow beat Illinois two times in this event.

Jim Shonerd of Baseball America noted that Jay didn't need long to show the scouts what they came to see: 

Illinois led the Big Ten in ERA and batting average and has the fewest losses in Division I baseball this season. The Fighting Illini are the marquee attraction at this tournament whenever they take the field, and Thursday will be no different.

Duchene commented on his team's position, per Mark Tupper of the Herald & Review:

Truthfully, I feel like we’re riding a wave. And there’s no reason to switch gears and get off that wave yet.

What’s going on is something we’ll be able to look back on in a couple of years and say, "Wow! It’s pretty amazing what we did." But it’s hard to look back on something when you’re living in the moment.

Maryland will at least pose a challenge, though.

The Terrapins knocked off Michigan State Wednesday, and the Spartans were the last team to defeat Illinois (all the way back in March). D1Baseball.com also projects Maryland as an NCAA tournament team.

A postseason win over Illinois would solidify an already solid Maryland resume and set the Terrapins up for continued success in the Big Ten tournament with a perfect 2-0 start in a double-elimination format.

The only problem for Maryland is the juggernaut standing in the way Thursday.
