Study examines minority physicians' role in care of underserved patients

Nonwhite physicians cared for 53.5 percent of minority patients and 70.4 percent of non-English speaking patients in an analysis of medical providers and their role in the care of underserved patients, according to a research letter by Lyndonna M. Marrast, M.D., of the Cambridge Health Alliance, Cambridge, Mass., and colleagues. Researchers analyzed data from 7,070 adults in the 2010 Medical Expenditure Panel Survey who identified a medical provider.

We're coming up on the new year, so it's time for new year's resolutions. For this week's Community Blog Topic we ask, "What are your World of Warcraft resolutions?"

I've got two characters tottering on the brink of 90, waiting for me to take them all the way. So I definitely want to get those taken care of. As far as pet battles, I want to get one of each kind to 25 and get as many related achievements as possible.

And then there's LFR. I have a confession to make: I've never used the Raid Finder. I fear PUGs. But I need to get over that while there are still people running them.

The Price Is Right Audi R8 Spyder

Of course, we love to tune into The Price Is Right to see average Americans spin the big wheel, play a round of Plinko or Cliff Hangers, and participate in the gladiatorial Showcase Showdown. But there is one reason, and one reason alone that the daytime game show has captivated us since its inception in 1972... 'A new car!!!'

Back in November, the show held a Dream Car Week that saw contestants get the chance to win whips like a Porsche 911 and a BMW 6 Series, amongst others. It must have been pretty successful, as the PIR team is back at it, this time giving away an Audi R8 Spyder with a sticker price of $157,300. That makes it the single most-expensive item given away in the show's history.

The R8's new owner, Sheree Heil, may not fit the image of a typical R8 driver - we anticipate she'll learn which side of the car to drive on after the show is over ­- but she is clearly tickled pink with her big win. Check out the insanity below.

Mercedes-Benz C-Class with AMG Pack

The new 2014 Mercedes-Benz C-Class is upon us, launching initially in North America with turbocharged four-cylinder C300 4Matic and six-cylinder C400 4Matic flavors. But that's only the tip of the proverbial iceberg when it comes to the vast array of powertrain configurations that will be offered in the new C-Class in markets around the world. Orders will soon be rolling in for gasoline, hybrid and diesel engines with four, six or eight cylinders, driving the rear wheels or all four, with the seven-speed automatic transmission soon to be replaced by a new nine-speed unit. But what enthusiasts are really looking forward to is the next C63 AMG.

As BMW has done with the M3 (and new M4), Mercedes has gradually ratcheted up the cylinder count in its AMG C-Class, graduating from the 3.6-liter V6 in the original C36 AMG to the 4.3-liter V8 in the C43 AMG, then the 5.5-liter V8 in the C55 AMG before going the distance with the sublime 6.2-liter V8 in the C63 AMG. Like its rivals, Mercedes is expected to use turbochargers as a replacement for displacement in the next model, but unlike its Bavarian rival, it won't be losing any cylinders in the process.

Skipping the 5.5-liter twin-turbo V8 that has gone on to power other AMG models, the next C63 AMG is still expected to introduce a new 4.0-liter twin-turbo V8 producing between 450 and 500 horsepower. It'll also reportedly keep the same seven-speed automatic transmission (instead of switching to the new nine-speed), but the jury's still out on whether it'll come with rear- or all-wheel drive (or offer buyers the choice). The downsized V8 - codenamed M177 - is then expected to find its way into other models, but the C-Class will be the first to get it when it arrives before the end of the new year ahead.

Another Week 17 Loss Would Be Fitting End to Jason Garrett Era in Dallas

This is Dallas—the football capital of the world. These are the Cowboys—supposedly "America's Team." Cowboys head coach Jason Garrett has some legitimate excuses for why his team has now missed the playoffs four consecutive years, but none of them should save his job. 

Not in Dallas. Not with a team that has this much talent and expectations that are as high as this one. 

I don't expect Cowboys owner/general manager Jerry Jones to fire Garrett, because he has seemingly gone out of his way to establish that the 47-year-old will get to coach this team for a fourth straight season in 2014. And he reportedly did so again after Sunday's season-ending loss to the NFC East rival Philadelphia Eagles.

But that's mystifying. 

The Cowboys have elite players at quarterback, running back, wide receiver, tight end, left tackle, defensive end and linebacker.

They didn't have that quarterback Sunday night, but they did have Tony Romo in 15 of their 16 games this year. Every other key offensive player was in the lineup. 

They didn't have that elite linebacker Sunday night, but they did have Sean Lee in 11 of their 16 games this year. DeMarcus Ware and should-be Pro Bowler Jason Hatcher were there, as was the $50 million Brandon Carr and last year's purported first-round gem, Morris Claiborne. 

Yes, the Cowboys were hit harder by injuries than their opponent Sunday night. And yes, they were in worse shape than the majority of the league for much of the year. But the Green Bay Packers didn't have their All-Universe quarterback for nearly half the season. They're in the playoffs. As are the Cincinnati Bengals, who were destroyed by injuries in 2013. As are the New England Patriots, sans—at various times—Rob Gronkowski, Aaron Hernandez, Jerod Mayo, Vince Wilfork, Devin McCourty and Nate Solder. 

That's why Garrett's firing shouldn't be based solely on this 24-22 Week 17 loss to the Eagles, but the defeat should act as confirmation. It should be the final straw. It should have been viewed as Garrett's final chance to save his job, however stacked the odds against him were. 

"I thought our team really fought," said Garrett after the game, per NFL Network, "and battled hard and overcame a lot of different obstacles to get to this point and...to give ourselves a hell of a chance to win at the end."

Absolutely, these guys certainly seem to like Garrett, and they never stopped playing for him. But that isn't enough. And close is nice, but this isn't horseshoes. 

Garrett runs an offense that continually ignores the run. He gambled by having backup quarterback Kyle Orton drop back to throw 46 freakin' passes against the Eagles. He gambled by once again underutilizing DeMarco Murray, who took only 17 handoffs despite entering Week 17 as arguably the league's hottest running back. 

He gambled by going for it on 4th-and-1 from the Philly 40-yard line in the fourth quarter, and he gambled on that play itself by having Orton throw. 

And he gambled by having Orton make a low-percentage throw on that fateful two-point conversion attempt late in the fourth quarter. 

None of those gambles paid off. And so while his and his players' final efforts were admirable, it just wasn't enough. Not when we're talking about the future of a $1 billion organization—one which won't have made the playoffs for half a decade by the time it gets another shot next year. 

It's time to start over. Jones might be too deeply embedded to see it. He might be too enchanted by Garrett's obedience to care. The problem might thus be Jones himself, but there's nothing any of us can do about that. He paid for this team and doesn't have to answer to anybody. 

Cowboys fans will just have to hope that Jones has an epiphany and finally sees what many of us outsiders have realized for weeks, if not months or years—that Jason Garrett isn't the man to lead this franchise out of a rut that has seen it go 136-136 with just one playoff victory over the last 17 years.

Germline genetic testing for familial cancer syndromes is usually performed serially for the most likely genetic causes. In recent years the way genetic testing carried out has changed, as next generation sequencing now allows the simultaneous testing of multiple susceptibility genes at low costs.Case presentationHere, we present a female with bilateral breast cancer and endometrial adenocarcinoma. After simultaneous sequencing of 150 genes (890 kb) associated with hereditary cancer we identified pathogenic mutations in two high-penetrance genes, i.e. TP53 and CDH1 that would most likely not have been elucidated by serial screening of candidate genes.
Conclusion:
As the two mutated genes are located on different chromosomes and cause different cancer syndromes these findings had a tremendous impact not only on genetic counseling of the index patient and her family but also on subsequent surveillance strategies.

In what we can only presume/hope is a precursor to the launch of the next-gen WRX STI, Subaru has released three videos now giving us a look back at the history of the STI-badged WRX models. In this third video, Dominick Infante, national manager of product communications at Subaru of America, got ahold of the automaker's 2011 WRX STI that broke records for the Isle of Man TT Challenge a couple of years ago.

In 2011, rally driver Mark Higgins piloted this car to a lap speed record of 115 miles per hour and a lap time record of 19:56.7 around the 37.8-mile course. The race-prepped STI, which Infante refers to as an "STI on steroids," features a beefed-up suspension, a wonderful-sounding open exhaust and the removal of the speed limiter to go with full roll cage and racing seats and harnesses.

Premier League Week 19 Fixtures: TV Schedule, Live Stream Info and Predictions

There have been times in the 2013-14 Premier League season that neutrals have simply not known which way to turn, with shock results and classic encounters commonplace over the first 18 games.

The traditional Boxing Day calendar of fixtures was no different and, with no pause for thought, we accelerate to the coming weekend and the mid-point of the league campaign.

Live Stream Information

This weekend sees four games slated for live coverage on Sky Sports, the broadcaster dominating the holiday period.

All Sky fixtures are available to subscribers via their television service on channel 401 or 402, while games can also be viewed online via the Sky Go website.

The service can also be accessed via the Sky Go application on all compatible smartphones and hand-held devices.

Highlights will be available to all UK-based viewers on BBC One's Match of the Day programme on Saturday night, while Sunday's games will feature on BBC Two's MOTD2.

 

Game of the Week - Chelsea vs. Liverpool

There is only one possible choice for Game of the Week this time around, with title-chasing Liverpool and Chelsea coming head-to-head in West London in a game that will heavily impact the belief of both sides heading into the new year.

While Liverpool have been simply sensational at times this campaign, they lie a point behind Jose Mourinho's Chelsea at present—despite the Blues failing to hit top gear.

It is perhaps ominous for the rest of the league that Chelsea are performing quite so well without ever looking like a side equal to the sum of their parts, but they will be severely tested by Brendan Rodgers' Liverpool.

The Reds were unlucky in Boxing Day defeat to Manchester City, after which Rodgers voiced his displeasure with the referee in no uncertain terms post-match, per Sky Sports.

Likely still to be without Steven Gerrard, Liverpool have seen Joe Allen and Jordan Henderson really step up of late but still need them to prove themselves consistently able to influence high-pressure games.

Should they fail, allowing Oscar, Eden Hazard and other Chelsea attacking talent possession of the ball, it will prove a tough night in West London for Rodgers and his side.

Breakfast Topic: What do you want to see under the tree?

Grandfather Winter tends to bring some epic gifts. Whether we're talking about exploding cars or exploding balloons, there's always been something with a little bang and boom under the tree. But that makes me wonder what we should expect this year. In a world thoroughly soaked in pandas, beer, and scions of fallen warlords, what could be under the tree in 2013?

Of course, we know that Warlords of Draenor is lurking around the corner. Could there be a TARDIS? Maybe a DeLorean, ready to soak up the gigawatts and launch us into a new adventure? Or perhaps something a little more father afield: is this the year we see an exploding submarine?

Regardless of what it ends up being, we can always hope and dream. That's what the season is all about, after all. So, what do you hope to see under the tree this year? Let your imagination run wild and enjoy the wonderful, creative season for everything it's worth.

Spy Shots: Mercedes-Benz CLS63 AMG Shooting Brake looking fine with facelift

2014 Mercedes-Benz CLS63 AMG spied in camouflage

The Mercedes-Benz CLS63 AMG Shooting Brake has been prescribed both a face- and butt-lift to bring it in line with current AMG standards, and our spy shooters caught it recovering from the surgery. There will be a slightly curvier grille and larger intakes on the front spoiler as on the S63 AMG, as well as a resculpted rear bumper and diffuser. We're told the LED units up front will be updated, too.

Other spy shots show an interior reworked with the distinct COMAND screen setup that's still causing arguments as to whether its an improvement or not. Since the wagon was introduced this year and its 5.5 liter twin-turbo V8 was just updated, there aren't changes expected under the hood.

"Invincible" and other great WoW arrangements by June Lee


We know some of you prefer to play with the game sound muted, but we're big fans of the World of Warcraft soundtrack -- and if you are, too, we think you'll enjoy June Lee's WoW arrangements. We've talked about Lee's excellent arrangements of WoW tunes before, and we continue to be impressed by his latest -- a string quartet playing Wrath of the Lich King's "Invincible." However, if strings just aren't your thing, earlier this month he posted a piano arrangement of "Invincible." If you like what you hear, Lee has a bunch of other instrumental arrangements on his YouTube channel, all of which impress.

Although superficial vein thrombosis (SVT) has traditionally been viewed as a benign disease, it is associated with an increased risk of deep-vein thrombosis (DVT). However, there are almost no data to suggest which patients may be at increased risk of subsequent DVT following SVT. In this week's issue of Blood, Roach and colleagues analyze results from a case-controlled study of nearly 5,000 individuals with DVT and more than 5,700 controls to determine how previous SVT could influence DVT risk in the setting of other risk factors.

UFC 168: Weidman vs. Silva Live Streaming Weigh-in Video

On Saturday at UFC 168, Anderson Silva will attempt to reclaim his middleweight crown from Chris Weidman.

In July, Weidman shocked Silva with a left hook that ended the Brazilian's record UFC championship reign. Because he was so dominant for so long, "The Spider" was given an immediate rematch and will try to avenge his first UFC loss.

Additionally, Ronda Rousey and Miesha Tate will meet in a rematch with a title on the line over the weekend.

The female bantamweights first met under the Strikeforce banner, with Rousey stopping Tate via her signature armbar. In order to take back her spot atop the women's 135-pound class, Tate will have to become the first fighter to avoid being submitted by Rousey in the first round.

The UFC 168 weigh-ins will be held on Friday at 7 p.m. EST. At that time, live streaming video will be available on the above video player.

Below is the entire fight card for UFC 168, which will be held at the Mandalay Bay Events Center in Las Vegas.

Hearthstone test season 1 ending January 2

Hearthstone Community Manager Keganbe chimed in to remind everyone that the first Ranked Play season will be ending soon. After the season ends, everyone's rank will be reset to 25 and the climb to the top will begin anew. We saw the fall (or at least, rebalancing) of mages in the inaugural season of Hearthstone. Test Season 2 will begin on January 2nd. We'll start seeing alternate card backs and golden heroes soon!
Hearthstone Test Season 1 End Reminder

Thank you for helping us test our very first Ranked Play season! This season is intended to conclude on January 2nd, so be sure to wrap up your climb up the ladder before that date. Once the season is over, everyone's ranking will be reset back to Rank 25, and Test Season 2 will begin!

During the course of this first test season we've gotten some very valuable feedback and data from you regarding the new Ranked Play system. We greatly appreciate your efforts in helping us test this updated Play mode, and we look forward to seeing your future feedback in Test Season 2. In the future, once the Test Seasons have concluded, we look forward to adding our Ranked Play rewards: Alternate card backs and Golden Heroes. We can't move on without your help!

The highest rank I've been able to reach so far is rank 8. How have your experiences in Hearthstone been? Which decks have you been using to climb the ranks? Personally, one month per season is too short. I think each season should be expanded to at least 2-3 months at most. I feel as though it takes quite a bit of time for constructed (play mode) players to get to their appropriate ranking.
